Application Process

If your organization has received approval to submit a full grant application,  please email the completed application to info@fannylandwirthfoundation.org.  You will receive a confirmation that your application has been received and will be reviewed at the next meeting of the Board of Trustees.  

Part A – Organizational Checklist

Part C – Proposal Narrative

Maximum of three single-spaced pages (Times New Roman 11pt font, 1” margins) that includes answers to the following questions:

Executive Summary (maximum 250 words) that includes:

1.    Grant amount requested (and for what program if applicable), organization’s mission, a summary list of your most important programs or activities, and types of clients served.

Grant Narrative (maximum 1750 words)

  1. Describe your most important programs (three maximum) – including their scope, their outcomes in the past 12 months, and what makes your delivery of those programs exceptional.
  2. What are your organization’s one or two most important strategic initiatives for the next 12 months that you hope will improve the organization’s impact on your clients (these need not be new initiatives or programs)?
  3. What are your organization’s biggest challenges (in addition to adequate funding) and how are you addressing them?
  4. What has your organization done in the past 12 months to strengthen your internal capacity to make your organization more effective? Are there other initiatives planned for the future?
  5. What is your organization most proud of and why?

Part D – Year-end Grant Follow-Up Report Questions

  1. How is your organizational story different from what it was one year ago? How did the FLF grant affect that story?
  2. Did you have any unexpected detours or challenges along the way? If so, how were you able to get back on track and what did you learn in the process?
  3. What does the path ahead look like for next year?

The applicant organization will be notified of the Foundation’s Board of Director’s decision concerning the grant request within two weeks following the Board meeting at which the request is considered.  Please email completed application to info@fannylandwirthfoundation.org.
